Important Dates
In order to make the registration process as simple as possible,
we ask that you complete and return all forms as soon as possible,
and at least by June 15. Final payments are also
due by June 15. We cannot guarantee acceptance of your camper(s)
if all forms and fees are not received by June 15 at the camp
registrar's address.

forms. All are writable .pdf files that allow you to type the
information directly into the file. Please send the completed forms

This provides general information on registration,
contact numbers, and lists all of the required forms to complete the
registration packet.

This lists the activities
that may be offered during camp in 2008.

This handbook is designed to help you prepare your child
for his or her stay here at camp this summer.
Registration Packet Forms
2008 Registration
Form
This is the official registration form for 2008.
Health Form
All insurance information must be completed.
A health physical and doctor's signature is required for
all campers and staff for 2008.
Camper Release
Form
This designates who will pick up your child at the
end of the camp week.
Parent
Questionaire
This helps our staff get acquainted with your child's
needs and interests.
Top Bunk Release
Form
This form gives or denies permission for the
camper to sleep on the top bunk bed.
Tuck Shop
Donation Form
This specifies the amount enclosed for your child's
tuck shop account. Any left over proceeds at the end can be donated to
Camp Cherith in Pennsylvania.
Challenge Course
Release Form
Camp Cherith’s challenge program integrates a variety of activities
that include warm-ups, group initiatives, low challenge elements,
zip line, and other challenge elements which may be physically
vigorous for the participants. We recommend that every camper have
a signed release form in case they want to participate in an activity.

Activity Selection Sheet**
Only complete this if your child will have
completed grades 1 or 2 by June 2008.
Pathfinder Activity Selection Sheet**
Only complete this if your child will have
completed grades 3 or 4 by June 2008.
Trailblazer Activity Selection Sheet**
Only complete this if your child will have
completed grades 5 or 6 by June 2008.
Challengers and Explorers Activity Selection Sheet**
Only complete this if your child will have
completed grades 7 or higher by June 2008.
** Activity assignments are made on the basis of this selection sheet.
Activities are assigned on a first come, first served basis, according to
receipt of this form and deposit. We try to accomodate the top choices but
cannot guarantee that they will always be available. Failure to return
the form means we guess what your child will enjoy!
